\u017ce zmiana\u201d realizuje zadanie publiczne \u201eAsystent osobisty osoby z niepe\u0142nosprawno\u015bci\u0105\u201d dla Organizacji Pozarz\u0105dowych \u2013 edycja 2026 przy wsparciu Ministerstwa Rodziny, Pracy i Polityki Spo\u0142ecznej.<\/p>\n<p>G\u0142\u00f3wnym celem Programu jest wprowadzenie us\u0142ugi asystencji osobistej jako formy og\u00f3lnodost\u0119pnego wsparcia w wykonywaniu codziennych czynno\u015bci oraz funkcjonowaniu\u00a0<br \/>w \u017cyciu spo\u0142ecznym, kt\u00f3rych adresatami s\u0105:<\/p>\n<ul>\n<li>Dzieci do 16 roku \u017cycia z orzeczeniem o niepe\u0142nosprawno\u015bci \u0142\u0105cznie ze wskazaniami: konieczno\u015bci sta\u0142ej lub d\u0142ugotrwa\u0142ej opieki lub pomocy innej osoby w zwi\u0105zku\u00a0ze znacznie ograniczon\u0105 mo\u017cliwo\u015bci\u0105 samodzielnej egzystencji oraz konieczno\u015bci sta\u0142ego wsp\u00f3\u0142udzia\u0142u na co dzie\u0144 opiekuna dziecka w procesie jego leczenia, rehabilitacji i edukacji.<\/li>\n<li>Osoby niepe\u0142nosprawne posiadaj\u0105ce orzeczenie:<\/li>\n<\/ul>\n<ol>\n<li>O znacznym stopniu niepe\u0142nosprawno\u015bci albo<\/li>\n<li>O umiarkowanym stopniu niepe\u0142nosprawno\u015bci albo traktowane na r\u00f3wni<br \/>z orzeczeniami wymienionymi w lit. a i b, zgodnie z art. 5 i art. 62 ustawy z dnia 27 sierpnia 1997 r. o rehabilitacji zawodowej i spo\u0142ecznej oraz zatrudnianiu os\u00f3b niepe\u0142nosprawnych<\/li>\n<\/ol>\n<p>Us\u0142ugi b\u0119d\u0105 realizowane na terenie miasta Cz\u0119stochowa oraz miasta Przyr\u00f3w przez wykwalifikowanych asystent\u00f3w, zgodnie z zapisami programu \u201eAsystent osobisty osoby <br \/>z niepe\u0142nosprawno\u015bci\u0105\u201d dla Organizacji Pozarz\u0105dowych \u2013 edycja 2026.<\/p>\n<p>Wsparciem obj\u0119tych zostanie oko\u0142o 50 os\u00f3b z niepe\u0142nosprawno\u015bci\u0105, w tym minimum 70% os\u00f3b z niepe\u0142nosprawno\u015bci\u0105 w stopniu znacznym oraz jedno dziecko do 16 roku \u017cycia\u00a0z orzeczeniem o niepe\u0142nosprawno\u015bci \u0142\u0105cznie ze wskazaniami: konieczno\u015bci sta\u0142ej lub d\u0142ugotrwa\u0142ej opieki lub pomocy innej osoby w zwi\u0105zku ze znacznie ograniczon\u0105 mo\u017cliwo\u015bci\u0105 samodzielnej egzystencji oraz konieczno\u015bci sta\u0142ego wsp\u00f3\u0142udzia\u0142u na co dzie\u0144 opiekuna dziecka
